Our Services

We base the vast majority of our business on reputation, word of mouth and having the right connections and this places us in a strategic position, with access to intimate knowledge of the industry and most importantly, knowing the right people. This can be used to ensure a smooth transition process for the potential resident or a patient moving into care, by linking them with the right facility.

It is also a physically, mentally and emotionally draining time for family members. Often at a time like this, not knowing what to do, where to go or who to turn to can be particularly daunting. In most cases they haven't had to go through this process before. As there are so many options out there, locating an appropriate facility is quite time consuming and the family's attention can often be shifted away from the immediate care of their relative. Our commitment to and knowledge of the industry enables us to help find the best and most appropriate care facility for the person and to remove the stress and uncertainty for family members allowing them time to regain their priorities.

A successful placement outcome is creating a smooth transition for the potential resident and their family when the person is discharged into care. This involves choosing an appropriate facility that would suit the individual needs of the potential resident. Even if the facility is deemed appropriate to meet the needs of the resident, a very important element is that they feel good about the facility they choose. We then take care of all administrative paperwork and help the resident move in. Our placement service consists of two key areas: discharge planning and case management.

Discharge planning services

Through our discharge planning service, we work with hospitals to ensure patients have a place to go as soon as they are discharged into care at a nursing home. We also work with the aged care facilities to ensure they maintain a healthy waiting list. Ultimately we endeavour to create a situation from which both parties benefit.

Case management services

In the aged care industry every case is different. At Health and Aged Assist we treat each case separately and go the extra mile to ensure all of the resident's individual needs are met. This may involve taking on the role of a de-facto family member who goes shopping for the person or providing them with emotional support. Moving into an aged care facility is a huge step and often it is met with mixed emotions from both the resident and their families. It can be particularly challenging if the resident has dementia, behavioural problems or is frail aged and is in need of special attention. In this case we ensure the placement process is managed in the most sensitive, effective and efficient way possible.

What makes a good placement

There are three main components that constitute a good placement:

  1. Finding the right facility. Just like finding the right school, car or home, choosing the right facility is a life changing decision. The right facility needs to cater for the individual care needs of the resident, both in the short term and long term.
  2. Geographic preferences. Placing the resident in a location they are familiar with will enable them to maintain their network of friends and encourage visitation. Keeping them sociable will increase their morale and self-esteem.
  3. Financials. Having the right financial advice is very important. Placing a relative into care can often cause additional stress for the families, because of the 'means testing' and the enormous financial commitment. At Health and Aged Assist, we will arrange for a licensed aged care financial adviser to provide the right financial advice.
