Aged Care Private Case Management

If one is waiting for a funded case management package and is in need of case management immediately, Health and Aged Assist can provide bridging case management until the funded package becomes available.
The private case management packages are available to
- Frail aged
- People suffering from dementia
- People with behavioural issues
- Until funded case management becomes available
- Until the client needs to enter residential care
- Full 'care need' analysis
- Identifying an individual's care needs
- Working with other health professionals to provide the necessary care
- Organising where applicable, activities, day care program, residential respite, mobility equipment etc
- Accessing and utilising the various community services available
- Liaising with and reporting to family members or nominated representatives
- Plus any other services required to meet an individual's care needs
If the person's family is unable to keep watch on a relative in residential care or at home, Health and Aged Assist is able to help with all facets of personal care, ranging from shopping, banking or regular visits and companionship. A full regular report would be forwarded on to the family or representatives.
